Which boundary matters
The operation boundary. In an estate that has been running for years, a change applied without a record is the single most reliable source of an unexplained outage.
The connectivity boundary matters next, but chiefly for partitioning rather than for availability: capacity shared between business units needs rules, or one unit's plateau becomes another unit's ceiling.
And the evidence question runs across all four. A control that works but cannot be demonstrated is one that will have to be defended from memory.
How the layers are set
Concurrency is partitioned with a guaranteed floor and an enforced ceiling per site or business unit, so behaviour at the limit is decided at design rather than discovered during a peak.
Every change is requested, reviewed, staged, verified and recorded with before and after state, and the record is produced as a by-product rather than assembled the week before an audit.
Administrative access is role-based and individually attributable, reviewed periodically, with permissions no longer required withdrawn rather than left in place.
